IMPORTANT: Complete this form only if you make a payment by Certified Official Bank Check or Bank Transfer. All Credit Card payments must be made through the AHFE Registration System

Please notice that completing this form without submitting a payment or contacting the conference administrator will not serve as an evidence of registration. This form is intended only for AHFE Participant Information Registration who have submitted their payment by Bank transfer or Certified Official Bank Checks. If paying by official bank check, please mail your check which is drawn on U.S. banks only, to the conference administrator at the address provided by AHFE Admin:

Please notify by email and send a copy of payment confirmation and the type of registration (e.g Early Registration, Best Deal, ...etc.) to: registration@ahfe.org

Notice: If your payment is for more than one person, EACH individual should complete a participant information form. If someone else is paying your registration fee, please be sure to include their name on the form in the requested area. This allows us to match the payment with your registration information. Student Participants whom register and pay "Student Rates" must submit an official letter (in English) from their University or School, duly signed and stamped, indicating their current student status. This document must be submitted when registering. Please send a scanned copy of this letter as soon as you register to registration@ahfe.org

Preferred Attendance Option




To give you more flexibility, we are offering two (2) attendance options

One paper included at no extra charge with registration. If you are the sole (only) author and have more than one paper submission then you can request a permission for extra papers to be published for standard fee per paper up to three papers. If you have multiple submissions, with multiple authors, a different author must register for each submission.
